If you are new to Theory & Practice, please print, complete, and return the following forms prior to your initial consultation. We asked that these forms be received in our office no later than two business days prior to your scheduled appointment. We also encourage you to provide all previous testing, school records, or other relevant information for our review.

You may also scan and email the forms in a PDF file to info@theoryandpracticereno.com. However, please note that because email is not a secure form of communication, confidentiality cannot be guaranteed.
